Job Description

Position Description: To provide assistance and support to program participants in mitigating barriers to medical care in accordance with Agency policies and procedure.

Essential Duties:

  1. Demonstrates knowledge of principles of client advocacy, cultural sensitivity, motivational interviewing, health education and HIPAA guidelines while providing assistance and support to program participants in mitigating barriers to medical care.
  2. Assess and document program participants’ home environment and compliance with the medical regimen.
  3. Develops and maintains knowledge of health, social and community resources and connects program participants with appropriate resources.

Other Duties:

1. Assists program participants with completion of forms for community resources and additional services.

2. Assists program participants with scheduling and attending medical visits.

3. Resolves difficult situations and provides a high level of service to program participants.

4. Provides regular updates on participant status and progress to the Nurse Navigator and other members of the healthcare team.

5.  Participates in interdisciplinary team conferences and provides input for development of the plan of care.

6.  Completes records and reports necessary for program service and administrative requirements.

7.  Establishes and maintains effective working relationships with program participants, families, peers, supervisors and community agencies.

8.  Complies with Agency Personnel Policies and Procedures.

9.  Performs other related duties as requested.

 

 

 
 

Job Requirements

Knowledge & Experience:

1. Knowledge of HIPAA and protected health information regulations.

2. Demonstrates effective Motivational Interviewing skills.

3. Effective oral and written communication skills

4. Ability to accurately document patient information.

5. A current driver PA driver’s license and a dependable automobile with appropriate insurance.

 

Education:

1.   High School Diploma or equivalent.

2.   Certificate of Completion for Community Health Workers Training Program.
