Job Description

Reports to:  Patient Care Manager (PCM)

Position Description: To provide care to the patient in the home.  In addition, the home health aide supplements the therapeutic and rehabilitative services of the professional staff as directed in the care plan, and in accordance with the policies and procedures of VNA Philadelphia.

Essential Duties:

  1. Observation, reporting and documentation of patient status and services delivered.
  1. Providing all aspects of personal care, including, but not limited to, services for: bathing, nail care, feeding, skin care,  toileting/elimination, oral hygiene, dressing, hair care and shampooing
  1. Assisting with rehabilitative and restorative activities under the direction of the therapist or nurse, which would include, but not limited to, the following activities: ambulation with or without assistive devices, transfer, re-positioning, range of motion, prescribed functional activities

Other Duties:

  1. Reading and recording temperature, pulse, and respiration.
  2. Measure and record input and output.
  3. Assist the patient with self-administration of medicine.
  4. Treatments requiring documented supervision by a professional which would include, but is not limited to, the following: enema, special skin care, non-sterile dressing and reinforcement of dressings, use of appliances and adaptive equipment, colostomy care.
  5. Nutritional assistance, including: meal planning and preparation according to diet instructions, serving and feeding, monitoring/encouraging/restricting fluid intake as instructed.
  6. Perform general housekeeping functions essential to patient care, which include: light cleaning of sickroom, light cleaning of bathroom, kitchen dishes, patient’s laundry, linen change
  7. Maintains effective working relationship with patient and family members.
  8. Records all activities performed and submits documentation as required.
  9. Communicates by phone any pertinent information to appropriate professional staff
  10. Performs relief visits/office work, if necessary, when there is a schedule opening.

Job Requirements

Knowledge & Experience:

  1. Must be a Certified Nursing Assistant by the Commonwealth of Pennsylvania and maintain valid PPD .
  2. Must have current CPR certification.
  3. A current driver PA driver’s license and a dependable automobile with appropriate insurance.
  4. 1- 2 years of current home care experience.
  5. Must posses a caring attitude towards the sick, is mature and exhibits mental and emotional stability.
