Job Description

Reports To Hospice Patient Care Manager

Position Summary: The LPN services in accordance with the policies and procedures of the Agency, under the supervision of a Registered Nurse and under the direction of a licensed physician.  Participates with the registered nurse in the assessment, planning, implementation and evaluation of nursing care.

Essential Functions:

  1. Provides supportive and restorative nursing care within the framework of care plan, health teaching and health counseling under the direction of a RN or licensed physician
  2. Observes, records & reports to the RN on the general physical & mental condition of patients.

Other Duties:

  1. Supports the Mission, Values and Vision of the Hospice of Philadelphia and the facility.
  2. Administers those prescribed treatments and medications as permitted by state law.
  3. Provides emotional and physical comfort of patients.
  4. Prepares equipment & materials for treatments, observing aseptic techniques as required.
  5. Assists in the physician and/or RN in performing specialized procedures to promote/maintain health.
  6. Assists the patient with activities of daily living, rehabilitative exercises as prescribed, and includes instruction in appropriate self-care techniques.
  7. Completes clinical and progress notes on a daily basis.
  8. Participates in in-service program, case conferences and seminars as directed.
  9. Provides direct patient care inclusive of treatments, medication administration, and patient education as ordered by the physician and directed by the RN.
  10. Reports patient condition changes to the RN and physician.
  11. Assumes responsibility for professional growth and maintains clinical competence.

Job Requirements

Knowledge and Experience:

  1. Registration as nurse in Commonwealth of Pennsylvania.
  2. One year’s experience in provision of relevant direct patient care.
  3. Possession of car, valid driver’s license, and auto insurance.
  4. Availability and ability to be on-call on a regular rotating basis.

Education: Graduate of an NLN-approved and state accredited program for nursing
