Job Description

Position Summary:  Works in collaboration with a physician and other members of interdisciplinary team (IDT), and palliative care team, in an active practice to deliver hospice, palliative care and related health care services to patients and families.

Essential Functions:

  • Treats Hospice and Palliative Care patients, including treatments and other therapeutic or corrective measures, within the scope of practice.  Completes comprehensive history taking, physical exams, and ordering tests, treatments and therapies for patients.  Prescribes medications as delegated and licensed.
  • Utilizes assessment, planning, intervention, and evaluation to implement IDT care plan, and serves as a resource to patients, their families and team members.

Other Duties:

  • Assist in all facets of care coordination for palliative care referrals.
  • Prepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement for services.
  • Comply with Collaborative Agreements and initiate reimbursement for services. 
  • Assist in development and/or revisions of clinical practice guidelines and standards for hospice and palliative care. 
  • Adhere to standards and practices of confidentiality regarding patients, families and VNA Hospice.  
  • Provides training and continuing education to staff.  Teaches and counsels family members, as well as patients.
  • Participates, as appropriate, in VNA Hospice educational activities, including new employee orientation and professional student clinical training and staff education programs.
  • Interprets Hospice and VNAGP services and programs to individuals/families and staff of other agencies.
  • Complies with VNAGP’s employee policies and procedures.
  • Complete other responsibilities as assigned.

Job Requirements

Knowledge and Experience:

  • Registration as Nurse Practitioner in the Commonwealth of Pennsylvania.
  • One year’s experience in provision of relevant direct patient care.
  • Valid driver’s license, and auto insurance.
  • DEA license and prescriptive authority.


  • Master’s Degree in Nursing with focus on Nurse Practitioner training and responsibilities.
