Job Description

Reports To:  Director, Rehabilitation Services

Position Summary: Develops, implements and assesses occupational therapy treatment programs for homebound patients.  Activities are conducted within the framework of Agency policies and procedures and are consistent with established professional standards.

Essential Functions:

  1. Manages a caseload giving consideration to patient/family needs, travel efficiency and service priorities.
  2. Plans, implements and evaluates individual occupational therapy programs and diagnostic tests in conjunction with the prescription and/or the direction of a physician.
  3. Treatment planning in consultation with the physician and interdisciplinary team (IDT) to achieve goals for clients and their families.

Other Duties:

  1. Instructs patients and family members in supportive, preventive and therapeutic procedures.
  2. Provides consultation to other Agency staff concerning the application of professional principles and procedures to problems in patient care.
  3. Consultation with IDT members.
  4. Identify barriers to desired outcomes.
  5. Evaluate equipment needs to increase functional level.
  6. Accountable to the Director of Rehabilitation for attainment of standards of professional practice.
  7. Completes records and reports necessary for patient service and administrative requirements as determined by the Agency.
  8. Assumes responsibility for professional growth and maintains clinical competence.
  9. Establishes and maintains effective working relationships with patient, families, peers, supervisors and community agencies.
  10. Responsible for meeting performance standards including qualitative and quantitative standards, as stated by the Agency.
  11. Identifies the need for services of other health care disciplines or Agencies and initiates the appropriate referral.
  12. Communicates regularly with the Patient Care Manager or  other health care personnel to develop coordinated plans for patient care.
  13. Supervises Home Health Aide in carrying out the established plan for care and evaluates the effectiveness of the service.
  14. Participates, as appropriate, in implementation of the Agency's Quality Improvement Program.
  15. Participates, as appropriate, in Agency educational activities, including new employee orientation and professional student clinical training and Agency staff education programs.
  16. Interprets Agency services and programs to individuals/families and personnel or other agencies.
  17. Participates as assigned in community programs.
  18. Carries out other activities relevant to therapy services.
  19. Complies with the Agency's Policies and Procedures. (Contract therapist also complies with contractual arrangement).
  20. Performs other related duties as requested.
 
 

Job Requirements

Knowledge and Experience:

  1. Possession of the appropriate licensure, certification and/or registration.
  2. Minimum of one year of direct care patient experience, preferably of a generalized nature. Current driver’s license.

Education:

Must have completed an educational program (Bachelors or Masters Degree) currently approved by the respective national professional organization
