Job Description

Reports To:  Patient Care Manager

Position Description The Wound Ostomy and Continence Nurse assists in the design, implementation, and evaluation of the VNA Philadelphia's Wound Ostomy and Continence Program.  In addition to providing direct nursing care services to appropriate patients, the Wound Ostomy and Continence Nurse is responsible to plan and administer educational programs for VNA Philadelphia's nursing staff to enhance their clinical skills in assessment, diagnosis, nursing care plan development, and patient evaluation for Geriatric and Adult Medical/Surgical/Wound Ostomy and Continence patients.  The Wound Ostomy and Continence Nurse is also responsible for developing, implementing, and educating staff and referral services on disease-specific protocols as requested.

Duties and Responsibilities:

  1. Assists in designing, implementation and evaluation of the wound ostomy and continence care program.
  2. Provides clinical expertise in wound ostomy and continence nursing to staff as needed.                
  3. Researches and develops patient and staff educational materials.
  4. Assists in organizing annual clinical education forum for Agency staff.               
  5. Develops community outreach program.
  6. Provides clinical field supervision and mentoring for Agency nurses.     
  7. Participates in interdisciplinary conferences to facilitate and coordinate patient plans and goals as needed.
  8. Identifies and develops in-services, in coordination with Director of Education.
  9. Designs and implements Wound Ostomy and Continence specific QI record review.                 
  10. Directs and participates in clinical research as related to VNAGP service.
  11. Designs and implements community education to promote the growth of the program.   
  12. Maintains professional education resources.       
  13. Assumes responsibility for professional growth and maintains clinical competence.     
  14. Meets all performance standards, including qualitative and quantitative standards, as stated by the Agency.
  15. Assists in the selection of equipment and in-service of all personnel who will use specialized equipment.
  16. Designs implements and evaluate clinical protocols for home care of patients with specific wound ostomy and continence diagnoses.         
  17. Reviews literature and contributes to Policies and Procedures updates as needed.
  18. Establishes clinical performance standards for staff, clinical competency testing and documentation for the Wound Ostomy and Continence Care Program.
  19. Performs other duties as assigned.
 
 

Job Requirements

Experience:

  1. RN with a minimum of three years wound ostomy and continence care experience.
  2. Excellent interpersonal and communication skills.
  3. Research and analytical ability and experience.

Education:  BSN with WOCN certification.
